This program provides Care Coordination Services to adults with mental, health-related disabilities, transportation, food, shelter concerns who do not qualify for Medicaid. To be eligible for this program an individual must live in Cattaraugus County, have significant mental health issues, and does not have Medicaid but can have Medicare or third party insurance. This program’s mission is to avert repeat hospital emergency room visits in Cattaraugus County.
