Essential Function

Responsible for driving individuals with a range of disabilities in a wheelchair accessible van to community locations. Also responsible for coordination of drop off and pick up times. Help to build program.

PT HHUNY Care Coordinator

Essential Function

Provide safe, non-judgmental, person centered, care coordination for persons with mental health and other disabilities to have full access to an array of services and assist the individual to meet their needs of community life, maintenance, and to attain/retain capability for maximum personal independence that fosters positive support. The Care Coordinator will be the hub of a care team connecting the individual to supports, services, and medical/psychological care that is needed. Care Coordinator will document all activities, develop care plans, do assessments, and provide referrals to consumers .

Community Habilitation Specialist

Essential Function

Responsible for assisting individuals to become more independent when performing activities of daily living to achieve their valued outcomes and personal goals. Habilitation services are designed to develop skills to increase an individual’s level of independence. Services are primarily community based and revolve around volunteer and recreational opportunities.
