Intake/Outreach Specialist
Essential Function
Complete all intakes for care coordination programs. Conduct outreach for the programs and process referrals as needed.

Essential Function
Responsible for assisting the Line Dance Program Director in: preparing, choreographing and teaching dance techniques for specialty dance classes such as line dancing to for individuals who are 12 and older, have a Developmental Disability, and are OPWDD eligible. Provides an enjoyable and safe dance program for program participants. Assists with lesson plans for students to correlate with students’ level of ability. Implements, choreographs and teaches dance routines when Line Dance Program Director cannot attend class. Demonstrates personal dance techniques skills and transfers to the student within their level of learning. Assist in communication with students and parents in regards to schedule, progress and performance. Maintains current and accurate records relative to all aspects of the program including. Uses company van to help transport individuals to and from classes for those individuals that need it.
