OPWDD Transition Support Specialist
Essential Function
Implement transition support services for youth and young adults ages 14-25 with developmental disabilities eligible through Office for People with Developmental Disabilities (OPWDD) living with their families in isolated rural communities. Help support individuals and families through the OPWDD eligibility process for those that are found not to be eligible for services.

Work with No Wrong Door (NWD) team to connect aging and disabled individuals and families to established support programs. Critical pathways providers to help facilitate access to needed home and community based supports and services, and streamline access to public benefits and application assistance. Also, provide Outreach working in conjunction with local NY Connects program for expansion and enhancement of NWD.
